Doctoral Program Forms
Students are responsible for initiating the filing process of all forms at the appropriate time in their program. They should obtain the forms, fill out the appropriate sections, take it to their faculty adviser for review and signature and submit them to the academic adviser for processing. Failure to submit the appropriate forms WILL stop a student from graduating.

This form appoints the doctoral student’s committee and documents the qualifying examination process. It should be filed with the journalism graduate studies center no later than the end of the student’s second semester of enrollment. [More]
This form confirms the student’s proposed plan of study. It MUST be submitted to the journalism graduate studies center by the end of the third semester. [More]
This form, specially designed for journalism, must be filed whenever a change is made in the student’s plan of study. Journalism requires that all program changes be approved by the entire committee. [More]
The purpose of this form is to record the official result of the doctoral comprehensive examination. Submit this form to the Graduate School within one month of exam completion. [More]
Students are required to gain approval of their committee as evidenced by the signatures on this form. The form must be filed no later than one year after successful defense of the comprehensive exam. [More]
This form reports the official results of the dissertation defense. The student should take this form to the defense, along with the dissertation approval page (printed on the required cotton bond paper) so that committee members can sign both forms for students who successfully defend their dissertation.
